Shell Cove Family Health uses a population health approach to identify local health needs and inform the development of focused and responsive primary care services.

  • High proportion of 0-4 aged children.
  • High proportion of single parent families supporting children under 15 years of age.
  • High unemployment rate.
HEALTH STATUS, EPIDEMIOLOGICAL AND HEALTH SERVICE NEEDS
  • High disease prevalence rates of:
      • Type 2 diabetes
      • Mental health conditions within females
      • Respiratory conditions especially COPD
      • Musculoskeletal conditions especially arthritis.
  • High rates of preventable mortality from deaths due to suicide and self-inflicted injuries.
  • High rates of hospitalisation for all six major causes:
      • Alcohol attributable
      • Smoking attributable
      • Stoke
      • High body mass index attributable
      • Diabetes
      • Ambulatory care sensitive conditions (preventable hospitalisations).
  • High mortality ratio figures for cancer and cardiovascular disease mortality.
